Black Hawk Center Assistive Devices & Equipment Loan Service




What is it?
The Equipment Loan Service is a service for people with disabilities and CIL consumers and a depository of used assistive devices and adaptive equipment such as wheelchairs (both power and manual chairs), walkers, crutches, shower chairs, hospital beds... etc. that the Center has in stock to send out on loan to those who make a request for an item.
All items in stock are donated by people in the community. If the CIL does not have an item it has yet to be donated. We would refer a consumer to the Used Equipment Referral Service (UERS) through Iowa Compass.
Who?
People with disabilities and Black Hawk CIL consumers can use this service.
Where?
This service is operated out of the CIL office.
Expectations and Requirements

What's it Cost?
Nothing! -- It's Free!
All items are free of charge. The only cost to the consumer is the cost of maintenance and repairs of the item while in possession of the consumer during the loan period.
How Do I Sign up?
Come to the Black Hawk CIL office or send a representative to pick out an item. The CIL is not responsible for transport of an item.
The consumer will be required to sign a lease agreement agreeing to maintain the item in good working condition and to return it when finished using the item.
